MEDICAL CERTIFICATE

Requesting medical evidence for your Mitigating Circumstances application?

If you need a medical certificate from us for your university's Mitigating Circumstances application, please contact us either in-person at reception, by phone, or submit a PATCHS request, to initiate the process.

We will ask you to complete a short questionnaire in order for the certificate to be issued. Depending on your situation, your GP might ask you to book a consultation appointment before issuing any certificate.

You need to be a registered patient for us to issue you a medical certificate.

If your need a GP consultation to discuss your health problems related to the Mitigating Circumstances application, please submit a PATCHS request to request an appointment.

Please be respectful of our GP's time and resources when requesting a medical certificate. As your GP practice, our priority is always to provide clinical care.

We have a different process for requesting medical evidence unrelated to the Mitigating Circumstances procedure, please see under 'Our Services' > 'Forms & Certificates': here
